Columbia Basin Health Association
It is CBHA’s mission to provide equal access to quality health care to all persons regardless of age, sex, color, ethnicity, national origin, or the ability to pay. We live out our mission statement everyday as we deliver our full range of services to people throughout central Washington. We came together, joined in a common purpose to provide health care to the most vulnerable and have created a primary care delivery system that is recognized as a leader in defining the health care home model.
Country Doctor Community Health Centers
Our mission is to improve the health of our community by providing high quality, caring, culturally appropriate primary health care that addresses the needs of people regardless of their ability to pay. At our Carolyn Downs and Country Doctor sites in Seattle, our providers, support staff and administrative employees all are committed to providing a humane, caring environment that respects our patient’s dignity, while acting both as the patient’s individual health advocate and as their advocate with the community at large to ensure that they receive the health care services they need.

Community Health Association of Spokane

Community Health of Snohomish County
Community Health Center of Snohomish County provides affordable medical and dental care. We welcome patients who are on Washington Apple Health, Medicaid, Qualified Health Plans (QHPs), Medicare, Medicare Advantage, Tricare, select private insurances and those who are uninsured. For those without insurance, CHC will help patients sign up for Washington Apple Health; if ineligible we offer a discount fee program. Our mission is to provide our diverse community with access to high quality, affordable primary health care.
Community Health Care
Community Health Care is a leading non-profit organization that offers quality health care to low income and uninsured families in the greater Tacoma area. We provide comprehensive family practice care for the entire family. With six Medical Clinics and two Dental Clinics throughout Pierce County, we seek to continuously improve our commitment and service to our patients and community. Community Health of Central Washington
Community Health of Central Washington is growing to meet the needs of our communities. Today, CHCW is the medical home for more than 30,000 people in central Washington. We provide comprehensive care for all ages, regardless of the ability to pay. Our patients receive preventive services, care for acute and chronic illness, and care in our clinics, hospitals and other sites. We provide prenatal and obstetric services in Yakima and Kittitas Counties.
Columbia Valley Community Health

Family Health Centers
Family Health Centers has locations in Bridgeport, Omak, Okanogan, Brewster, Oroville, Tonasket and Twisp. Each community health center is as unique as the town in which it is located, but all share the same mission: To provide high quality primary health care in the communities we serve regardless of the ability to pay. We are the only Joint Commission accredited health care provider in Okanogan County.

The Health Care for the Homeless Network (HCHN) provides health care services to people experiencing homelessness in King County and leadership to help change the conditions that deprive our neighbors of home and health.

International Community Health Services
Lake Roosevelt Community Health Centers
Lake Roosevelt Community Health Centers (LRCHC), is a tribal owned entity that serves the tribal communities of Inchelium and Keller in Ferry County as well as the surrounding areas off the reservation. Comprehensive programs and services are provided at these facilities and include the following: medical, dental, lab, radiology, podiatry, optometry, pharmacy, and physical therapy. The Tribes provide collaborative and complementary health services in the communities such as mental health, substance abuse, social services, community health, WIC, nutrition, maternal support services and community outreach which are co-located with the health services.
Mattawa Community Medical Clinic
Mattawa Community Medical Clinic serves the citizens in and around Mattawa, Desert Aire, Schawana, Beverly, and the Wahluke Slope communities. Our mission is to provide quality comprehensive healthcare for all. We are committed to staffing our program with culturally and technically competent people. Our goal is to identify, monitor, and serve the health needs of the community in order to improve health status.
Moses Lake Community Health Center
Moses Lake Community Health Center is dedicated to serving migrant and seasonal farm workers, the uninsured, under-insured and others who have difficulty accessing care. The Health Center has served the community in Moses Lake since 1978 and opened operations in Quincy in 2002.
The NATIVE Project
Our Mission is to provide quality services that promote wellness and balance of mind, body and spirit for individuals, staff, families and communities. We offer "Sacred Hospitality" to all people seeking services at The NATIVE Project. The NATIVE Project’s Service Delivery Philosophy is a patient centered care model that embraces action, education, culture and spirituality on the road to healing and wellness.
NeighborCare Health
We are the largest provider of primary medical, dental and behavioral health care services in the Seattle area focusing on low-income and uninsured families and individuals, seniors on fixed incomes, immigrants, and the homeless. Each year we care for about 65,000 patients at our 28 non-profit medical, dental and school-based clinics. We ask everyone to pay what they can, but no one is turned away due to inability to pay. With the help of our partners and donors, we work to make our city a healthier and safer place for everyone.
NEW Health Programs Association
NEW Health Programs Association is a not for profit, Federally Qualified Health Center (FQHC) that has proudly served the Eastern Washington Tri-County area since 1978. We believe that all people deserve access to healthcare regardless of their financial means or personal situation, and we pride ourselves on treating everyone with respect, integrity, and compassion within our patient centered medical home.
North Olympic Healthcare Network
The North Olympic Healthcare Network (NOHN), a federally-qualified community health center, provides high-quality, full-spectrum Primary Care, Behavioral Health, and Oral Health services to meet the needs of the North Olympic Peninsula. NOHN provides safe, effective, patient-centered, continuously measured and improved health care to every patient regardless of age, gender, race, creed, national origin, insurance status or ability to pay.
Peninsula Community Health Services
Peninsula Community Health Services (PCHS) is a community based nonprofit organization that offers quality, affordable care for children and adults in the Kitsap and Olympic Peninsula areas. Our services include primary medical care, dental care, behavioral health counseling, pharmacy services, referrals to specialty providers, and health education and promotion activities.
Sea Mar Community Health Centers
Sea Mar Community Health Centers, founded in 1978, is a community-based organization committed to providing quality, comprehensive health, human, housing, educational and cultural services to diverse communities, specializing in service to Latinos in Washington state. Sea Mar proudly serves all persons without regard to race, ethnicity, immigration status, gender, or sexual orientation, and regardless of ability to pay for services. Sea Mar's network of services includes more than 90 medical, dental, and behavioral health clinics and a wide variety of nutritional, social, and educational services.
Seattle Indian Health Board
The Seattle Indian Health Board (SIHB) is a private, non-profit 501(c)3 organization incorporated in the State of Washington. We provide community health care and services targeting the urban American Indian and Alaska Native population in the greater Seattle/King County area. Although some services and programs are offered to eligible Native patients only, most primary care services are available to everyone regardless of race, color, sex, national origin, disability, religion, sexual orientation or inability to pay. The Seattle Indian Health Board serves all patients and offers discounted fees to those who qualify.

Valley View Health Center
As one of the fastest growing community health centers in the region, Valley View Health Center offers primary medical, pediatric, dental, pharmacy, and behavioral health services, with a number of our clinics structured after an integrated model to health care service delivery. The continued growth of Valley View Health Center is illustrated by the recent addition of a 340B pharmacy to our Chehalis Clinic in January 2016. Further plans for expansion include adding pediatric dental services to our Pediatric Clinic, and adult dental services for our Olympia patients.
Yakima Neighborhood Health Services
Established in 1975, Yakima Neighborhood Health Services today serves more than 23,000 patients annually. We were the first Health Center in the state to achieve the highest level of recognition possible as a Patient-Centered Medical Home and we are accredited by the Joint Commission.
Yakima Valley Farm Workers Clinic
Yakima Valley Farm Workers Clinic serves over 141,000 people across 19 medical clinics, 10 dental clinics, and 57 programs in two states. We have expanded medical care to include dental, orthodontia, nutrition counseling, autism screening, and behavioral health services. We also offer community programs that offer assistance with employment and training, after-school education, rental and shelter assistance, energy assistance, weatherization, HIV and AIDS counseling and testing, home visits, tobacco cessation, and three mobile medical/dental clinics.
