Building Bridges to Respond to SDOH

A presentation by the Association of Asian Pacific Community Health Organizations (AAPCHO) on “Building Bridges to Respond to SDOH.” This presentation explores how health centers can build and sustain partnerships with community-based organizations to address clients’ social needs. AAPCHO will highlight the importance of enabling services (ES) data collection utilizing a national standardized tool, Protocol for Responding to and Assessing Patients' Assets, Risks, and Experiences (PRAPARE), and share strategies for CHCs to connect with other local agencies, ensuring better resource sharing, strengthened network of services, and improved collaboration to respond to SDOH. International Community Health Services (ICHS) will showcase examples of how they collaborate with CBOs to tackle SDOH and share their approach to providing enabling services. 




Intended Audience:

Intended audience includes community health centers’ staff working to address clients’ social needs, health equity, clinical, community health workers, social workers, quality improvement, etc.


Learning Objectives:

  • Learn promising practices to build partnerships between community organizations to address social needs

  • Understand how to use the PRAPARE toolkit to support their mission to respond to SDOH

  • Develop strategies for closing the loop on SDOH referrals
